Pre-registration Confirmation

Please note: Pre-registration does not guarantee you will be offered the COVID-19 vaccination. The vaccination is allocated per the Federal Government, the Pennsylvania State Department of Health and Philadelphia Department of Public Health. This survey is to verify interest and allocation needed only. Once the state allocates vaccinations to Centennial Pharmacy Services, Centennial will contact your organization’s point of contact to schedule the clinic and team members per regulated guidelines. At that point, your organization will send you a vaccination registration form, which must be completed prior to your scheduled vaccination. Always consult with your healthcare provider to identify if the COVID-19 vaccination is right for you, or if you have any conditions that may preclude receiving the vaccine.
